Passive transfer of Covid-19 immunoglobulin via breast milk post Covid vaccination of lactating mother: case report and review of literature

Abstract
Infant response against infections depends largely on active and passive transfer of immunity through breast milk. There is plenty of data showing Infant response against infections depends largely on active and passive transfer of immunity through breast milk. There is plenty of data showing transplacental transfer of anti-SARS-CoV-2 antibodies to fetus and presence of neutralizing Ig-A and Ig-G anti-SARS-CoV-2 antibodies in breast milk of mother post Covid vaccination. There is paucity of research regarding impact of Covid-19 vaccination of lactating mothers on immune response in infant blood. Index case was delivered at 35 weeks of gestation with features of hydrops fetalis. Covid Rapid Antigen test and Covid RTPCR test were negative for mother and neonate. Baby was investigated for sepsis and underwent double volume exchange transfusion and discharged on exclusive breast feed. SARS-CoV-2 antibody titres were tested in blood and breast milk samples four weeks after 1st and 2nd dose of Covid-19 vaccination of mother. Growth, neurodevelopment and hematological parameters were monitored over time. Robust immune response was reported in blood of infant post Covid-19 vaccination of mother with potential to confer passive immunity to baby and without any serious side effects in mother-infant dyad.
